Time and resources have been specifically reserved for your office visit(s) and/or diagnostic test(s).  There will be a charge of up to $150.00 if you fail to come in for a scheduled appointment or cancel/reschedule with less than 48 hours notice.  This fee is not covered by insurance and is your personal responsibility.
